Subject: Referral for Transplant cases — standard instructions in HMIS based digital referral protocol

An instance regarding referral for a transplant case given by a Railway Hospital was found to have been issued mentioning “Referred procedure/ treatment:…workup…” and “Type of Service: treatment/ procedure (outdoor) (approved session:1 no.)”. The ambiguity/ incongruity caused considerable confusion and inconvenience to the patient and the empanelled hospital.

2. Transplant cases, being highly complex, constitute a unique category of referrals. Unlike other referrals, all transplant cases essentially undergo four broad steps/processes viz. Workup tests → Medico-legal/NOTTO compliance → Transplant procedure → Follow-up. Workup tests are a complex battery of tests on the patient /donor spread over several sessions over a period of time. The transplant procedure is dependent on and decided based on the workup tests. Hence, the locus of workup or transplant procedure cannot be at different empanelled HCOs and must necessarily be covered under a single comprehensive referral.

3. In order to preclude repeat of such instances or error in issuing such referrals, the following has been incorporated as standard/dropdown auto-select menu in the HMIS referral/doctors’ desk (from where digital referral is generated in HMIS) and should be ensured/rechecked in all transplant referral cases:

Specialty: (to be picked from drop down menu in HMIS e.g., kidney/liver/lungs etc.)

Referred procedure/treatment: “Transplant with workup” (drop down menu in HMIS)

Referring doctor’s remarks: (clinical remarks to be recorded).

Type of service: (auto select in HMIS, if referred procedure/treatment is as per ii. above) –“OPD/IPD for comprehensive treatment including follow up”.[note: Kidney transplant cases would be inclusive of the dialysis required during the entire treatment period.]

Footnote in all transplant cases: Applicable CGHS package and due compliance with medico-legal/NOTTO requirements to be ensured by the empanelled HCO. Dialysis for kidney transplant cases may be billed separately as per CGHS rates (if not a part of the transplant package rate) or the patient may claim reimbursement for such dialysis.

This issues with the approval of DG(RHS).

विषय: प्रत्यारोपण (Transplant) मामलों के लिए संदर्भ (Referral) – HMIS आधारित डिजिटल रेफरल प्रोटोकॉल में मानक निर्देश।

 

यह पाया गया है कि रेलवे अस्पताल द्वारा एक प्रत्यारोपण मामले में जारी रेफरल में “Referred procedure/treatment: … workup …” तथा “Type of Service: treatment/procedure (outdoor) (approved session: 1 no.)” का उल्लेख किया गया था। इस प्रकार की अस्पष्टता/असंगति के कारण रोगी तथा पैनल में शामिल अस्पताल को काफी भ्रम और असुविधा का सामना करना पड़ा।

प्रत्यारोपण के मामले अत्यंत जटिल होते हैं और इन्हें एक विशिष्ट श्रेणी के रेफरल के रूप में देखा जाता है। अन्य रेफरल के विपरीत, सभी प्रत्यारोपण मामलों में सामान्यतः निम्नलिखित चार व्यापक चरण/प्रक्रियाएँ शामिल होती हैं:
वर्कअप जाँच → मेडिको-लीगल/NOTTO अनुपालन → प्रत्यारोपण प्रक्रिया → फॉलो-अप।

वर्कअप जाँच में रोगी/दाता पर अनेक जटिल परीक्षण शामिल होते हैं, जो विभिन्न सत्रों में और एक निश्चित अवधि तक किए जाते हैं। प्रत्यारोपण प्रक्रिया का निर्णय इन्हीं वर्कअप जाँचों के आधार पर किया जाता है। अतः वर्कअप और प्रत्यारोपण प्रक्रिया अलग-अलग पैनलबद्ध स्वास्थ्य देखभाल संगठनों (HCOs) में नहीं हो सकतीं और इन्हें एक ही समग्र (Comprehensive) रेफरल के अंतर्गत शामिल किया जाना अनिवार्य है।

भविष्य में ऐसी त्रुटियों या असंगतियों की पुनरावृत्ति को रोकने हेतु, HMIS रेफरल/डॉक्टर डेस्क (जहाँ से HMIS में डिजिटल रेफरल जारी किया जाता है) में निम्नलिखित को मानक/ड्रॉप-डाउन ऑटो-सेलेक्ट मेनू के रूप में शामिल किया गया है। यह सुनिश्चित किया जाए कि सभी प्रत्यारोपण रेफरल मामलों में निम्नलिखित का सही चयन/पुनः जाँच की जाए:

Specialty (विशेषज्ञता): HMIS के ड्रॉप-डाउन मेनू से चयन किया जाए (जैसे – किडनी/लीवर/लंग्स आदि)।

Referred procedure/treatment: “Transplant with workup” (HMIS के ड्रॉप-डाउन मेनू से)।

Referring doctor’s remarks: चिकित्सकीय टिप्पणियाँ/क्लिनिकल विवरण अंकित किए जाएँ।

Type of Service: (यदि उपर्युक्त ii के अनुसार रेफरल किया गया है, तो HMIS में ऑटो-सेलेक्ट होगा) –
“OPD/IPD for comprehensive treatment including follow-up”।
(नोट: किडनी प्रत्यारोपण के मामलों में उपचार की पूरी अवधि के दौरान आवश्यक डायलिसिस भी शामिल रहेगा।)

सभी प्रत्यारोपण मामलों में फुटनोट:
संबंधित CGHS पैकेज लागू होगा तथा मेडिको-लीगल/NOTTO आवश्यकताओं का अनुपालन पैनलबद्ध HCO द्वारा सुनिश्चित किया जाएगा।
किडनी प्रत्यारोपण के मामलों में डायलिसिस का बिल CGHS दरों के अनुसार अलग से प्रस्तुत किया जा सकता है (यदि यह प्रत्यारोपण पैकेज का हिस्सा नहीं है) अथवा रोगी द्वारा ऐसे डायलिसिस के लिए प्रतिपूर्ति (Reimbursement) का दावा किया जा सकता है।

यह आदेश DG(RHS) की स्वीकृति से जारी किया गया है।
